A failed AI evaluation needs a destination more useful than a reporting channel. Someone must be able to change what the product is allowed to do.
Suppose an assistant proposes account-record changes. In an illustrative test, it confuses two accounts with similar names. Whether that failure blocks release depends on the workflow. A person may verify identity before any change, or the assistant may already have permission to execute. The same model error carries different consequences.
Write a response card for that failure category:
Consequence: a change could affect the wrong account.
Evidence: retain the failing input, expected behavior and exact candidate configuration.
Immediate restriction: remove automatic execution for the affected operation while preserving reviewed suggestions, if that reduced workflow is acceptable.
Authority: name the role that can impose the restriction and the role that can authorize restoring execution.
Then test the restriction. An owner who can request a change but cannot make it take effect does not yet have an operating response. The control may need to remove a permission, disable an action path, or route the work through review. Its behavior matters more than the label on the escalation document.
The shared evaluation team still has a necessary job. It can challenge the test method, preserve comparable versions and identify failures across products. The domain owner explains why a particular confusion matters. The release owner determines which remaining behavior the evidence supports. Those responsibilities can sit with a small number of people without disappearing into one generic approval.
For consequential uses, independent review can help expose assumptions the delivery team is under pressure to accept. It should contribute a decision, such as a required restriction or unresolved failure category. Adding reviewers who cannot alter the outcome creates another meeting, not another control.
The boundary also works in the other direction. A product owner cannot accept an organization-wide risk merely because a local workflow appears useful. That consequence needs the appropriate authority and a defined state while escalation is pending.
Before adding the next metric, trace the most consequential failed case through this path. Confirm who reads it, what they can restrict, how that restriction takes effect, and what evidence would justify removing it. The evaluation becomes operational when an inconvenient result can change the product.
